The AB CD approach discriminates between the classes of drugs for use as monotherapy using several sources of evidence: trials comparing the blood pressure lowering effect of different classes of drug [474], drug rotation studies [472, 475] and renin profiling [476, 477, 478, 479]. Age and ethnicity are argued to have major roles in the blood pressure lowering efficacy of antihypertensive drugs. Younger Caucasians usually produce higher renin levels, have renin dependent hypertension, and thus respond better to first line therapy with drugs which suppress the renin system - those belonging to the A or B class. Older participants and patients of African origin, with low renin hypertension, respond better to drugs in class C or D [473]. Class C and D drugs activate the renin system making second line therapy with a drug which suppresses renin activity more effective. Hence, when dual drug therapy is required, it is plausible to combine drugs from classes A or B with C or D, which have a complementary action on the renin system [470, 471, 472, 473]. Secondary analysis of the LIFE trial [457, 458] generated concern that -blockers alone, or combined with diuretics, in elderly patients may lead to a greater incidence of diabetes mellitus than other drugs although it is not strictly clear whether the combination increases the risk of diabetes or is less preventative than other drugs ; . Pharmacologically the effect is plausible since -blockers are observed to increase insulin resistance and thiazides reduce insulin output in some patients. A combination of the two agents could have a synergistic negative effect. This has led to a recommendation for dual therapy in elderly patients to be restricted to combining class A with an initial drug from class C or D [473]. Consequently, a combination of classes A, C and D has been recommended for triple therapy. Remaining licensed drugs for treatment resistant hypertension include spironolactone, -blockers and centrally acting antihypertensives. To inform the concern about combining a thiazide and beta-blocker, data on the incidence of diabetes was abstracted from drug trials using this combination. Seven trials reported this data see Table 30 ; . Treatment in comparison arms involved initial treatment with a newer antihypertensive drug with subsequent addition of a thiazide diuretic, beta-blocker or other dugs if necessary. We estimated a pooled risk ratio for new onset diabetes using a random effects model. The definition of new-onset diabetes used in most trial reports was unclear. The proportion of patients exposed to a thiazide diuretic - beta-blocker combination varied and was poorly reported: no estimation was possible for the CAPPP trial. Time exposed to drug combinations was unreported and we assumed that second line therapies were initiated without substantial delay. Overall there was a significantly higher incidence of diabetes in patients randomised to beta-blockers or thiazide diuretics of whom about half received both agents: RR alternative vs. thiazide diuretic-betablocker combination 0.81, 95%CI: 0.77 to 0.86 see and actimmune.

It has been demonstrated that the lactobacilli found in a sizable sampling of salivas comprised a spectrum of species of which 56 per cent of the isolates were homofermentative and 44 per cent were heterofermentative. Also, homofermentative species were isolated from 94 per cent and heterofermentative species from 75 per cent of the positive samples. At least two species were present simultaneously in 60 per cent and three species in 23 per cent of the samples. Curran, Rogers, and Whittier 1933 ; found that more than half of their dental strains were L. casei and that the oral lactobacilli available to them were not usually L. acidophilus. Pederson personal communication ; has emphasized to us that many of the oral strains he was able to study are heterofermentative. Harrison 1939 ; observed gas in 33 per cent of his cultures. Although Bergey 1923 ; had divided the genus on the basis of homo- and heterofermentation and Orla-Jensen 1919 ; had even created separate genera for the two types, dental investigators before 1950 did not take advantage of this natural approach for the differentiation of species. Tilden and Svec 1950 ; in a preliminary report identified 58 of 101 oral strains. Forty-five were L. casei and 13 were L. ferinenti. They stated that none of their cultures resembled L. acidophilus Bergey Breed et al., 1948 ; and as also defined by Anderson and Rettger 1937 ; , Weinstein, Anderson, and Rettger 1933 ; , OrlaJensen 1919, 1943 ; , Orla-Jensen, Orla-Jensen, and Winther 1936 ; , and Tittsler, Geib, and Rogosa 1947 ; . Their special medium for isolation may not have been especially favorable for the growth of the latter species.
