Key words. Phenolphthalein ; chemical pneumonia ; PET scan ; aspiration pneumonia ; lobectomy. Abstract. A 76-year-old female, presented with a recurrent cough and haemoptysis after an aspiration of a `grain de Valls'. Chest radiography and computed tomography demonstrated an atelectasis of the medial segment of the right middle pulmonary lobe. Bronchoscopy was negative for any malignancy but the onco-PET-scan showed a pathologic hypermetabolic lesion in the right middle lobe. A lobectomy was performed and pathology revealed a `destroyed lung' with images of foreign body reactions and microabscesses without any sign of malignancy. Postoperatively, clinical improvement with complete resolution of the symptoms was observed. With this case report we demonstrate the irritative and clastogenic characteristics of phenolphtalein, the main active ingredient of the `grain de Valls'. This case also illustrates an example of a false positive PET-scan.

As percutaneous exposures comprise the greatest risk of transmission of blood-borne pathogens, avoiding percutaneous exposures should be a primary concern to the DHCP. The careful handling of needles and other sharps is an important aspect of avoiding percutaneous exposures. Engineering controls and work-practice controls for needle handling safety are of particular importance to prevent percutaneous exposures. Aspirating anesthetic syringes, and selfsheathing needles should be considered for routine use. Newer designs should be considered as they become available. Work-practice controls for needles and other sharps would include: Used disposable syringes and needles, scalpel blades and other sharp items should be placed in appropriate puncture-resistant containers located as close as feasible to where the items were used. Needles should be capped prior to use. Needles should not be bent or otherwise manipulated by hand, or handled so that they are pointed towards any part of a DHCP'body. s Used needles should not be recapped by hand. To the Editor: Oremek and Sieffert [1] suggested that extensive physical activity increases prostate-specific antigen PSA ; concentrations. They demonstrated the increase after a standardized exercise on a cycloergometer for 15 min at 75100 W in many healthy men with different ages. We performed PSA measurements during various sport performances to establish if strenuous exercise increases the serum PSA in young men. We measured PSA on 30 men with a mean age of 27 years range 18 45 ; participating in ultraendurance performances of different sports. Twelve men participated in the Pine 24-h, an ` ultraendurance nonstop 24-h speed ice skating competition February 1994, Italy three in the PaviaVenezia, an ultraendurance water ski race of 450 km April 1995, Italy 10 in the Fila Sky Marathon, a marathon performed at an altitude of 4300 m October 1995, Himalaya, People's Republic of China and five in the Millegrobbe, a 3-day cross-country ski competition of 100 km January 1995, Italy ; . Service d'Imagerie Mdicale - Facult de Mdecine Vtrinaire de Lige - Belgium Introduction Purposes: Tears of the cranial ligament of the medial meniscus have been reported in the horse. The diagnosis of cranial meniscal ligament injury is mainly made by arthroscopy, a radiographic diagnosis being possible only in chronic cases with bony changes on the proximal tibia. The aims of this presentation are: 1 ; to illustrate the ultrasonographic technique to assess meniscal cranial ligaments in the standing horse, 2 ; to describe the ultrasonographic anatomy of the cranial meniscal attachments, 3 ; to present abnormal images obtained in clinical cases with cranial meniscal ligament injury. Materials & Methods: Ultrasonography was performed on the flexed stifle using a 7.5 MHz linear transducer. No stand-off pad was used. Two normal isolated limbs and 5 sound horses were used to establish the normal ultrasonographic features of the cranial meniscal ligaments. Abnormal images were obtained on clinical cases with stifle lameness. Results: The cranial attachments of the menisci can be assessed ultrasonographically on standing, non sedated, horses. Medial meniscus cranial ligament is easier to image compared to the lateral meniscus cranial ligament. On transverse ultrasonographic images the medial meniscus cranial ligament shows a regular fibers' alignment and appears homogeneously echogenic. Its insertion on the proximal tibia appears as a smooth, slightly concave, hyperechoic line. Abnormalities which can be detected by ultrasonography include hypoechoic areas within the ligament, irregularities of the tibial insertion surface and bony fragments localized in the area of the attachment. Conclusion: Ultrasonography appears to be a valuable imaging modality for a non-invasive assessment of cranial meniscal ligaments. Cranial meniscal ligaments evaluation should be included in the routine ultrasonographic examination of the equine stifle and ultrasonography should be used to confirm radiographic findings suggestive of cranial ligament insertion injury and lumigan.

Studies of other potential novel therapies for ALI, including activated protein C and alveolar fluid clearance with - agonists, are in progress 88 ; , as are studies of early goal-directed therapy for sepsis. Although the direct applicability of the results from such studies to A H5N1 ; disease is uncertain, they may suggest interventions of possible benefit and identify those of unlikely utility in this disease. Until such data become available, it is recommended that agents of unproven value not be used in the treatment of A H5N1 ; disease and mecamylamine.
