BACKGROUND Most side effects and other health problems associated with the use of Norplant implants are not serious. As mentioned previously, changes in menstrual bleeding patterns are by far the most common adverse effect. In addition to menstrual bleeding changes, women using Norplant implants occasionally develop enlarged ovarian follicles. Fortunately, they rarely cause symptoms and usually are discovered only incidentally at pelvic examinations. In addition, they generally shrink and disappear spontaneously and rarely require treatment. Ectopic pregnancies also have occurred although clinical studies have shown no increase in the rate of ectopic pregnancies per year among implants users compared with users of no contraceptive method. Finally, several other conditions which may or may not be associated with use of implants have been reported. They include headache, breast tenderness and or discharge, weight gain, increased body or facial hair hirsutism ; and vaginal infection vaginitis ; . In this chapter, additional information and guidelines for assessing and managing the most important of these side effects and other health problems are provided. MENSTRUAL BLEEDING CHANGES The most frequently reported side effect is a change in the menstrual bleeding pattern. Because the changes vary widely, it cannot be predicted what kind of change a particular client may experience. If increased frequency of bleeding occurs, the quantity of blood lost rarely is enough to cause anemia, but there have been a few cases which required treatment with iron tablets. Fortunately, these bleeding problems gradually diminish over time i.e., after 6 to 12 months ; . Despite the fact that medical treatment for irregular bleeding or spotting usually is not necessary, many clinicians feel obligated to give some medication in an attempt to correct the altered bleeding pattern and to improve continuation. Multiple clinical studies have shown the oils of fatty fish, EPA and DHA, to be of benefit to the cardiovascular system, brain and visual function, immune function, skin health, inflammatory modulation and joint health.25-26 As ALA conversion to EPA is poor to moderate, depending on many variables, and conversion to DHA is minimal, it is generally thought that regular twice-weekly fatty fish oil consumption, or fish oil supplementation, is the best way to enhance EPA DHA status.27 Several factors inhibit conversion via elongation enzymes ; of short-chain ALA to its long-chain metabolites, EPA and DHA including: a diet high in omega-6 EFAs, saturated fats and trans fats all common in the U.S ; , which can compete with the enzymes needed to convert ALA to EPA; high alcohol intake; a deficiency of vitamins B3, B6, C and zinc or magnesium, which are required by elongase and delta-6 desaturase conversion enzymes; loss of delta-6 desaturase activity that occurs during normal aging; certain health conditions, such as diabetes, RA, viral infections and drugs medications; and certain populations, such as Native Americans, Inuit, Asians, Norwegians and Welsh-Irish may not effectively convert ALA to EPA in the body, as they come from a genetic history of high fatty-fish intake.

There are currently 15 different antiretroviral drugs approved by the FDA for use in HIV-positive individuals. Before 1995 there was only one class of drugs available called Nucleoside Reverse Transcriptase Inhibitors NRTIs ; . Drugs in this class were generally used as monotherapy one single drug taken several times a day ; or occasionally in combination with another NRTI. In 1995 and 1996, two new classes of drugs were introduced: Protease Inhibitors PIs ; and Non-nucleoside Reverse Transcriptase Inhibitors NNRTIs ; , also known as "non-nukes." The availability of these new agents dramatically changed the way in which HIV was treated. PIs and NNRTIs began being used with the original class of drugs, NRTIs, in triple combination therapy, sometimes referred to as drug "cocktails." This usage eventually came to be known as Highly Active AntiRetroviral Therapy, or HAART. According to the U.S. Department of Health's federal guidelines for the treatment of HIV infection, HAART should be a combination of at least three drugs with demonstrated potency. 11 13 Semaforului St., RO - 550309 Sibiu; e-mail: ana benedek yahoo The present study, carried out in 2000-05, in some massifs of the Southern Romanian Carpathians namely Retezat, arcu-Godeanu and Lotru Mountains ; , aimed to reveal and put forward some hypothesis on the dynamics of the rodent communities, both in space and time. A special interest was paid to the relations between rodents and insectivores populations' dynamics. In Retezat Mountains 20 stations and about 40 habitats and in arcu-Godeanu 10 stations and 20 habitats were researched at different heights and in different habitat types, during the summer - autumn months. In Lotru Mountains 5 stations were established along a transect from 600 up to 2000 m altitude. The lower stations were researched both in the warm and in the cold season, while in the higher stations investigations were carried on only during summer. Investigations were carried out by live-trapping of rodents using Longworth-like wooden box traps. The traps were set either in a rectangular net in forests ; or in transect along river banks ; , at 10 m distance one from another. They were checked twice a day, in the night and at dawn. Captured animals were determined based on external morphological features. They were marked by cutting their fur in different parts of the body, and then released. The rodent's community from the Southern Romanian Carpathians is dominated by the two typical forest species: Apodemus flavicollis and Clethrionomys glareolus. Specific diversity and abundance of rodents' communities decrease on altitude. The highest number of species is found in the lower areas, in the beech forests zone, while in the alpine level, above the upper limit of the subalpine shrubs, only 3 rodent species were recorded Marmota marmota - only in Retezat Mountains, Pitymys subterraneus and Chionomys nivalis ; . The impoverishment of small mammals' communities at higher altitudes is recorded both in years with low and high populational densities. However, some factors can influence the normal altitudinal distribution. Thus, due to the habitat disturbance by timber exploitation at lower altitudes, in the year 2003 in arcu-Godeanu Mountains no significant numerical increase of the community was observed here. The seasonal dynamics of rodent populations was registered in one station from Lotru Mountains at 850 m a.s.l. on Cailor Rivulet ; . The study revealed a discontinuous presence of the typical forest rodent Apodemus flavicollis in the area. The period of its presence in the monitored habitat is confined to the warm season, but it varies greatly from year to year. In 2000 this species was captured until November, while in 2001 no individual was found after August. In 2001 the first specimen was captured in May, while in 2004 the species was already present in February. Downstream on the same valley, this rodent recorded in winter relative high capture frequencies. This result leads to the assumption that vertical migrations take place during the year, probably along the river. A. flavicollis breeds at.
