Because of the differences in CsA dose response between investigators using the IPK as a model of acute CsA nephrotoxicity, we postulated that different perfusate constituents may have been responsible for the differences in CsA dose response. There is no evidence from the studies reported above that perfusate constituents albumin alone, with serum, or with whole blood ; had a significant effect on the expression of CsA toxicity in this model. In all three types of perfusate, the effect of CsA, with methanol as vehicle, was modest and comparable; a concentration of 25, 000 ng mL produced a 60% reduction in GFR. We suspect that the CsA vasoconstnictive effect may have been blunted by the use of methanol as the vehicle, because methanol caused significant vasodilation in this model see CsA Vehicles below ; . To our knowledge. there are no reports of the use of fresh whole rat blood in the IPK; washed human red blood cells have been used in previous studies where blood was included in the perfusate 8 ; . Whole rat blood was used in these studies because renal vascular tone, distal nephron integrity and function, and GFR were closer to normal with blood than without. It seems likely that the IPK is far closer to in viva conditions with perfusate that contains whole blood. The placement of a transfusion filter in the circuit should have ensured the removal of all cell aggregates and platelets from the perfusate. thus minimizing any contribution that may have had in the effects seen with vehicles, CsA, or metabolites. The results reported above on three different perfusates-albumin, albumin and serum, on albumin and whole blood-suggest that the discrepancy in CsA dose response between investigators using the IPK is probably not due to differences in perfusate composition.
Results: Ove rall, fourteen isolates were found to be resistant to va ncomycin. E . faecium was the only vancomycin resistant strains VRE ; isolated. MIC for these strains was 256. The results showed no difference in the PCR-restricted patt e rn of vanHAX cassette among the isolated stra i n s. Plasmid profiling also s h owed 3 different patterns of the isolates. Conclusion: The results showed low level vancomycin resistant among the gram positive strains isolated from out-patients in Iran. The VRE, on the other hand, showed a significant resistant to number of antibiotics examined here. Overall, the results suggested that VRE isolates were polyclonal. Further genotyping variation among the isolates is under investigation. They were afterwards improved by Dr. Maskelyne and Mr. Mason, and still more lately by Burg and Burckhardt; the error of these last tables will seldom exceed fifteen seconds, or seven miles and a half. The computations, however, necessary in making use of these tables, were found to be very laborious and inconvenient; to obviate this difficulty, the nautical almanack, suggested by Dr. Maskelyne, was published, which is now annually continued. The longitude is thus ascertained to such a nicety, as to secure the navigator from any danger arising from the former imperfect modes of finding it; "he is now enabled to make for his port without sailing into the parallel of latitude, and then, in the seaman's phrase, running down the port, on the parallel, as was done before this method was practised. Fifty years ago, navigators did not attempt to find their longitude at sea, unless by their reckoning, which was hardly ever to be depended on." Not long after the mariner's compass was employed, its variation was noticed; as it is obvious that, unless the degree and direction of this variation are accurately known, the compass would be of little service in navigation, the attention of navigators and philosophers was carefully directed to this point; and it was ascertained that the quantity of this variation is subject to regular periodical changes. By means, therefore, of a table indicating those changes, under different latitudes and longitudes, and of what are called variation charts, the uncertainty arising from them is in a great measure done away. Another source of error however existed, which does not seem to have been noticed till the period of Captain Cook's voyages: it was then found, "that the variation of the needle differed very sensibly on the same spot, with the different directions of the ship's head." Captain Flinders attributed this to the iron in the ship, and made a number of observations on the subject; these have been subsequently added to and corrected, so that at present the quantity of variation from this cause can be ascertained, and of course a proper allowance made for it. It does not appear that any material improvement has been made in the construction and use of the log, --that useful and necessary appendage to the compass, --since it was invented about the end of the sixteenth century. These are the most important improvements in nautical knowledge and science, which renders navigation at present so much more safe and expeditious than it formerly was; there are, however, other circumstances which tend to the same object; the more full, accurate, and minute knowledge of the prevalent winds at different times of the year, and in various parts of the ocean; the means of foretelling changes of weather; and, principally, a knowledge of the direction and force of the currents must be regarded as of essential advantage to the seaman. When to these we add, the coppering of ships, which was first practised about the year 1761, and other improvements in their built and rigging, we have enumerated the chief causes which enable a vessel to reach the East Indies in two-thirds of the time which was occupied in such a voyage half a century ago.

The workup process is a time when all the disciplines perform evaluations and conduct a multidisciplinary review. The inclusion criteria are evaluated for appropriateness and safety of the patient to undergo a BMT. Interventions: This preadmission process facilitates nursing care by ensuring: 1. Patients understand the physical, emotional and financial aspects of the treatment 2. All pre-admission diagnostic tests and procedures are performed 3. Opportunities for students to observe and perform patient-centered care in preparation for their Advanced Pharmacy Practice Experiences APPEs ; . Students receive smoking cessation and immunization administration training in the fall quarter of their first year and can immediately apply their knowledge and skills in a patient care setting. An innovative acute care IPPE for first year students was implemented in January 2007. Description: Students are paired with faculty preceptors on the Medicine or Cardiology services spending 2 hours per week for 10 weeks. Students interview patients identified as smokers to assess their readiness to quit. For patients ready to quit, students make appropriate pharmacotherapy recommendations and provide smoking cessation counseling. Students screen inpatient records or interview patients to determine whether pneumococcal and or influenza vaccines are indicated. The IPPE allows students to develop competencies in oral and written communication skills, cultural competence, problem solving skills, and professionalism. Upon completion of the experience students are required to write a reflection paper describing what they learned and how the experience has impacted their path to becoming a pharmacist. Results: Preliminary results of the IPPE have been positive for students and faculty. Implications: This is the first step in developing a continuum of IPPEs that will incorporate 1st, 2nd, and 3rd year students working together with 4th year APPE students and faculty preceptors. Our goal is to develop an integrated student IPPE model that can be used in a variety of settings that provide patient care. Midwestern University Chicago College of Pharmacy has identified ten global curricular outcomes. Three of the outcomes are: communicate effectively, act in a professionally responsible manner and promote such actions in others, and demonstrate self assessment and self-directed learning. These outcomes are embedded in every course in the curriculum, extracurricular activities, and the general college culture. However, the College saw the need to offer a special extracurricular program to a limited number of students to provide them with an additional opportunity to develop leadership skills. Thus the Student Leadership Institute SLI ; was created. This program is open to students at all the colleges of pharmacy serving the state of Illinois. It is a competitive application process, and students meet several times over the course of three months. The curriculum consists of several distinct modules: presentation skills, meeting facilitation skills, dealing with difficult people, Situational Leadership, DISC Principles, business etiquette, and networking tips. In addition, each student is matched with a pharmacy leader mentor and is required to shadow their mentor for 1 2 day to observe and participate in leadership activities. Students are given pre-post tests at every workshop, required to keep a reflective journal, and are required to present a careers in pharmacy talk at a local high school to practice skills learned in the SLI. Methods: For 3 years, the college has used a validated and reliable Student Rating system IDEA Center's Diagnostic Form ; to obtain feedback on instructors and courses. As part of the evaluation, instructors select broad learning objectives that are essential to their course. The extent to which those objectives are met is measured by student response to the evaluation. The 12 objectives are: * Gain factual knowledge * Learn fundamental principles, generalizations, theories * Learn to apply course material * Develop specific skills, competencies, points of view needed by professionals in the field most related to this course * Acquire skills in working with others in a team * Develop creative capacities * Gain a broader understanding and appreciation of intellectual cultural activity * Develop skills in expressing myself orally or in writing * Learn how to find and use resources for answering questions or solving problems * Develop a clearer understanding of, and commitment to, personal values * Learn to analyze and critically evaluate ideas, arguments, and points of view * Acquire an interest in learning more by asking questions and seeking answers Outcomes: Mapping these learning objectives supplements the mapping of our educational outcomes. Each objective was categorized using Bloom's, Krathwohl's, and Simpson's taxonomies. The objectives were assigned to various years of the curriculum according to the ``theme'' of each academic year pharmacy systems, drugs, simulated patients, patients ; and their hierarchy with the three learning taxonomies. In 2003, ``Professionalism'' was added as a tenth curricular goal; terminal and enabling objectives were written and were also assigned to various years of the curriculum. Since the 1997-98 academic year, students have been survey to determine whether they have been taught the material associated with each objective and whether they could perform what is described in each objective. The results of these surveys have been used to assess students' confidence in what they are learning in the curriculum and serves as one piece of information to assess curriculum effectiveness and the need for curricular change.
